Ice Forms 2007

Only Home Club and Associate Club members in good standing are allowed to book ice. You must book ice before the deadline date or pay a late fee as outlined on the form.  Any new members booking ice a late will not be charged. If space allows you may book in 1/2 sessions, please confirm this first before you book. 

You may leave your form with the rink monitor or mail them to APFSC P.O. Box 1075 Westfield, MA 01085.
